With over 18 years of experience as a fertility specialist, I have seen hundreds of women with PCOS successfully conceive—some naturally and others with assistance. In this blog, let's understand how PCOS affects fertility, what treatment options are available, and why there is still hope for those struggling to conceive.

What Is PCOS and How Does It Affect Fertility?

PCOS is a hormonal imbalance where the ovaries produce more male hormones (androgens) than normal. This disrupts the natural menstrual cycle and ovulation. Many women with PCOS may not ovulate regularly or at all, which is the main reason they find it difficult to conceive

In PCOS, small fluid-filled sacs (called follicles) develop in the ovaries, but they don't mature properly. As a result, the egg is not released on time or may not be released at all. Women with PCOS often experience irregular periods, weight gain, acne, excessive facial hair, and difficulty losing weight. Even women who appear fit and healthy may silently suffer from anovulation due to PCOS.

The good news is that with proper lifestyle changes and medical help, ovulation can be restored in many cases, making pregnancy possible.

Diagnosing PCOS: A Step Toward Clarity

Many women with PCOS go undiagnosed for years, especially if they are not trying to get pregnant. A diagnosis is usually made based on symptoms, blood hormone levels, and ultrasound findings. Some may have all the classic signs, while others have subtle symptoms.

At our center, we perform a thorough evaluation to understand the type and severity of PCOS in each patient. We check hormone levels like LH, FSH, AMH, insulin, and androgens and also assess thyroid and prolactin levels to rule out other causes. An ultrasound helps us see if the ovaries show the typical “string of pearls” pattern, which is a common feature of PCOS.

The goal is not just to give a diagnosis—but to understand how PCOS is impacting your fertility, your health, and your future family plans.

Treatment Options: A Personalized Approach

There is no one-size-fits-all treatment for PCOS. The choice of treatment depends on the patient's age, symptoms, fertility goals, and other health factors. For some, lifestyle changes like weight loss, healthy eating, and regular exercise can restore ovulation naturally. Even a 5-10% reduction in weight can make a big difference in hormone levels and fertility.

If lifestyle changes alone don't help, we use ovulation induction medications like Letrozole or Clomiphene to help stimulate egg release. These are often the first step in fertility treatment for PCOS. If that doesn't work, we may move on to gonadotropin injections or Intrauterine Insemination (IUI).

In cases where multiple attempts have failed or where other factors are also present, IVF (In-Vitro Fertilization) may be recommended. The IVF protocol is carefully customized to prevent complications like Ovarian Hyperstimulation Syndrome (OHSS), which PCOS patients are more prone to. With expert care, IVF success rates in PCOS patients can be very high.

It's important to remember that PCOS doesn't just affect your fertility. It can also increase your risk of diabetes, high cholesterol, heart disease, and endometrial problems. That's why managing PCOS is important even after pregnancy.
